First: Communication

This is a very important component to our positive partnership in this program. I often become overly involved in client’s lives and want to help day/night/weekends and holidays. I have learned over the past few years that this is not good for me or my family. So, I ask you to please communicate through the Dario app. This keeps me in balance as a mom and an educator. It also keeps all of our dialogue in one place, it can be really hard to help individuals when I am racing between texts, emails, CGM data and Dario data. So, please use the chat function through Dario for all communications.

I work M-F, 8-5 MST.

Second: Data Collection

Grant data. In order to show our need for funding and the value of our work, we have to show that our program improves distress around diabetes management as well as improved blood sugars.

So, please take a moment to fill out the following forms and update your A1C anytime you have results.

We utilize another platform, called Dario, to track blood sugars, insulin dosages and carbohydrate intake. This helps us to get a complete picture of your blood sugars, catch errors that could be contributing to your frustrations and empowers you to have better self-management of your sugars.

Individual Meetings

If you invested financially in the program even at the Tier 2 level, you are able to meet with me 1:1 every week. If you are doing the Tier 3 program, individual meetings are available to you at $25/session, which can be set up with me through my schedule below.

These meetings are your responsibility to schedule, if you do not schedule a meeting, I will presume that you do not need any additional guidance this week. There is a link in each weekly module to make that easy. There are times in CF when chaos arises unexpectedly, on both our ends. I just ask that you give me as much notice as you can if you need to cancel or reschedule.
